Having something on your site to attract the visitor's interest and keep them engaged can significantly increase traffic and conversion rates and interactive assessments are ideal for this purpose.
I must say this is a fantastic tool. It looks great, is easy to install - I had mine on my site within 20 minutes www.phobiaclinic.net/phobia-test.htm - it's completely customizable, and I can think of thousands of uses for all kinds of websites, not just for life coaches.
Having searched and searched for a simple self-assessment tool and found only complex and high cost solutions, this is the answer to my prayers.

This is the original option for people who want to install the assessment on their own site.
This is a 10 question 'Readiness for Coaching' questionnaire with three evaluation texts where all elements are fixed.

This is a 10 question questionnaire with four evaluation texts where the title, heading, questions and evaluation texts are fully customisable.
In addition, the score 'threshold' levels at which the various evaluation texts are displayed is also fully customisable, as is the appearance of the Submit and Print buttons, enabling you to create a wide range of assessments to suit your needs.

The assessments are written in Flash and are available as .swf files.
The Pre-defined version is fully self-contained in the .swf file.
The Custom and Custom Professional versions have associated text files for the assessment title, heading, questions/statements and evaluation texts that are supplied with the .swf as a self-extracting ZIP (.exe).
With the Custom and Custom Professional versions, extract the .swf file and associated text (.txt) files into a folder of your choice by double clicking the self-extracting executable (.exe) and clicking Browse to select the desired target folder.
Putting an interactive self-assessment on your site couldn't be easier.
Simply put the assessment (.swf file) and any associated text files into a folder on your site, create a web page in the same folder, insert the assessment (.swf file) into the page at the appropriate position within your text and index the page into your site.
You now have the interactive assessment on your site.
All that remains is to enter the text you want to appear in the assessment title, heading, questions/statements and evaluation texts into the relevant associated text files and away you go.
You can have as many different assessments on your site as you wish, the only restriction is that you cannot have more than one Custom or Custom Professional assessment on the same page as they use text files that have the same names, though it is possible to have one Custom and one Custom Professional on the same page as the text file names are different.
Clicking the 'Add To Cart' button will take you to the shopping cart where you can pay by credit card.
Once payment has been confirmed you will receive an e-mail thanking you for your purchase and containing a link to the download. At busy times this may take a few hours.
